What Is A Proper Work Up?
Doctor D does an excellent job of pointing out that more work up is not better in medicine and, in fact, can be dangerous. My favorite line is “Okay, let’s take a moment to consider a well-known but poorly-understood fact: doctors hurt people.” If you don’t Doctor D’s feed in your reader, I highly suggest you add it. He writes in a thoughtful, entertaining manner...

Stages of Growth of a Fibromyalgia Diagnostician
Stage 1 (medical student): OMG, there is no such thing as FM. You’re all just a bunch of whiny losers without evidence of real disease who want to be on disability. Stage 2 (Intern): Fine, “FM” is “real” but the treatment is to get off your fat butt and exercise daily. So don’t whine to me unless you’re doing that and don’t ask for disability...
Lonely technophile looking for hardbody EMR
I’m an admitted technophile. I couldn’t tell you a single NBA basketball player or who the in-couple in Hollywood is, but I’d geek out if I met Ryan Block or Peter Rojas on the street. I can write my own computer programs and I cut my teeth reallocating memory manually on a black and green screen DOS system to get it to play games from 5 1.4 inch floppies when I was 12. My reputation is...

Pundits and politicians act as if government can solve almost any problem. At the slightest hint of trouble, the ruling class reflexively assumes that knowledgeable, wise and public-spirited government regulators are capable of riding to the rescue.via townhall.com Posted via web from Curbside | Comment »

Karma's Biatch
Today I got consulted on an elderly lady with rash, fever, weight gain, dyspnea, fatigue, weakness, leukocytosis, and transaminitis. Her hospital course has also been complicated by HIT. She’s already been worked to the hilt at another academic tertiary care center that transferred her to us for a second opinion. I think the ferritin over 80K is going to be the clue that solves...

iPhone scrolling So a few weeks ago swissmiss tweeted about the fact that in almost any (all?) iPhone apps, in order to scroll all the way up to the top, instead of manually scrolling, you can just touch the time and it hyperscrolls to the very top. Since then, I’ve probably added 5 minutes of usable time to my life and respected the iPhone even more than I already do.via...
Perspective
I still need to learn to appreciate the easy consult as a specialist. Sure, a rheum consult to evaluate for SLE in someone with a negative ANA and with bacterial endocarditis may seem stupid to me, but it’s easy money so I should be grateful. Posted via email from Curbside | Comment »

H1N1 Flu (Swine Flu) Self Test →
jayparkinsonmd:
If you think that you may have swine flu, take this little test and hopefully save yourself an expensive visit to the doctor.

Responsibility
A gentleman failed to keep his appointment a second time for a hospital follow up but did manage to call afterwards to request refills. He hinted that he will probably not be able keep an appointment in the near future.
I encouraged him to check his email as there are plenty of offers there from people willing to sell him Viagra without an evaluation. If he wants continued care from me...
How to get a decent consult
If you want a useful consult, explicitly state why you want the consult.
In the first place, you’ll make your consultant happy for two reasons. One, a reason for consult is required by insurance companies for billing a consult. Two, it makes figuring out what role you want me to play easier and the visit better.
The second reason is you’ll make yourself happier. There are many...
